Free Perimenopause Symptom Tracker (Printable)

Last updated October 21, 2026. Educational tool only, not medical advice: see our Medical Disclaimer.

Perimenopause symptoms come and go, which makes them hard to describe in a 10-minute appointment. This free tracker lets you rate 12 common symptoms each day, mark your period days, and print a clear 30-day summary to take to your doctor. No sign-up is needed, and your entries stay in your own browser.

1. Log today

Rate each symptom: 0 = none, 1 = mild, 2 = moderate, 3 = severe.

2. Your last 30 days

Your entries are saved only in this browser, on this device. Nothing is sent to us.

How to Use the Tracker

  • Log once a day, ideally at the same time, such as before bed.
  • Mark your period days. Changes to your periods are usually one of the first signs of perimenopause, according to the NHS.
  • Track for 2 to 3 months if you can. Patterns over several cycles are far more useful than a single bad week.
  • Print or download it before your appointment, and add the questions you want to ask.

Why Track Your Symptoms?

Many perimenopause symptoms overlap with other common conditions, such as thyroid problems, low iron, stress and depression, so a record of when symptoms happen and how they relate to your cycle helps your doctor work out what is going on. The UK guideline from NICE says perimenopause can usually be identified from symptoms in women aged 45 or over who have hot flashes and a change in their periods, without blood tests. That makes a good symptom record especially useful.

When to See a Doctor Sooner

Don’t wait to finish tracking if you have:

  • Bleeding after 12 months or more without a period
  • Periods that are much heavier than before, or bleeding between periods
  • Perimenopause-like symptoms before age 40
  • A fast or irregular heartbeat, or symptoms that make it hard to get through the day
